Introduction: The basic activity of community health nursing is the care for the
individual, the family and the community. Community nurse first time enters in
the family when she visits a pregnant woman. Treatment continues with visits
of newborn and infant in the first year of life and monitoring puerperium in the
postpartum period. Preventive activities are intended to support the puerper-
ium in the care of newborn and monitoring the development of the newborn
before visiting the paediatrician. In the first year of the child's age community
nurse, perform preventive visits to families in order to promote and support
them during the period of child development milestones. In recent years, risk
factors have been identified by community nurses that indicate a higher vulner-
ability of this population.
Methods: In Health Centre Postojna we have approached to the implementa-
tion of national project activities that envisage the extension: I. the number of
visits to a new-born child where a nurse detects family elements of vulnerabil-
ity and II. substantive definition visits of puerperium and infant in the first year
of life. Project activities are carried out since June 2018. The article presents
the implementation of activities and the satisfaction of the family with the im-
plementation of the activity.
Results: Since the beginning of the project, all visits to newborns, infants and
puerperium have been carried out according to project activities. Additional
visits were made at 44.6% of all visits. The added content is also key to achiev-
ing vulnerable groups and facilitating access to health services for all.
Discussion and conclusion: It have turned out that additional content by users
is well received and that the frequency of treatments in vulnerable groups of
the population has increased.
